Hi,

I have patients with BCBS. Their claims failed to file and the reason is output exception… How do I correct this so I can refile correctly?

Hi ​@Ella Eyes! The output exception message is usually just an initial error code indicating a technical glitch with data formatting, coding errors (like wrong CPT/ICD codes), missing info, or system incompatibility. You should be able to click into the claim row and further review the “Failed Validations Box” which will list the reasons as to why the claim failed.
