Add an Addendum to an Initial Assessment Form

PERMISSIONS REQUIRED: Therapist can make Addendums for their assigned Cases, Administrators or higher can make Addendums for all Cases.

 

After an Initial Assessment is completed and signed, you can make an Addendum to that Initial Assessment if the Diagnosis has changed.

IMPORTANT: Any changes to diagnosis codes will not affect existing treatment plans, regardless if they are signed or unsigned. If you want the updated codes to display on the treatment plan you will need to manually link them.

 

Add Addendum 

  1. Locate Client and open Client Profile.
  2. Click Notes in left side menu. 
  3. Click the Initial Assessment and Diagnostic information tab.

    NOTE: You can prevent your staff from submitting and signing progress notes that do not have a diagnosis code entered. See Prevent Staff from Submitting/Signing Progress Note without a Diagnosis for more information.

  4. Click Addendums button to bring you to the Addendums section.

  5. Scroll down and click + Add Addendum button in the bottom right corner.

    NOTE: You cannot free text to enter new codes or edit existing codes. Only codified diagnosis codes will be available.

     

  6. In new Addendum screen, enter date, time, and notes.
  7. Click +Add Diagnosis or Edit to make diagnostic code changes. The Diagnosis codes pop-up opens.

  8. Click the trashcan icon to remove a Diagnosis.
  9. Click + Add diagnosis code to enter a new Diagnosis.
  10. Click Save.

  11. The Addendum displays at the bottom of the Initial Assessment.
