This article reviews the Transitions section. It assesses 18 different areas that can assist the IEP team in making transitions and educational placement decisions for a student (e.g., over Milestones and scores, social skills, group learning, independent work, self-care skills, adaptability to change).

Overview - Access Level

  1. Click the assessment date link on the main VB-MAPP page. The assessment opens.

    NOTE: Click the Information icon at the top of the assessment > Chapter 7 to access the official VB-MAPP guide for Transitions.

  2. Transitions is the second section on the left side of the screen. Click the Begin button to begin scoring the section or the Resume button to continue scoring an assessment in progress.

  3. There are 18 Transition measurements. Transition measurements assist the IEP team in making transitions and educational placement decisions for a student (e.g., over Milestones and Barriers scores, social skills, group learning, independent work, self-care skills, adaptability to change).

  4. Each measurement shows the scores entered for the student. Assessments are numbered in the grid (1st, 2nd, 3rd, 4th, etc.) and are based on the order they were entered on the main VB-MAPP table.
    For example, the score for the measurement (Overall VB-MAPP Milestones Assessment Score) for the 3rd assessment is 2.

    The score is then calculated into the total transitions score for the 3rd assessment that displays on the main table. When you open an assessment from the table it automatically opens that assessment.

Measurement Tabs

Each measurement has three tabs: Scoring, Definition, and Notes. If you need more info, Click each tab to see specific information.

  1. Scoring - point breakdown and explanation for scoring the measurement.
    Possible points - 1 Points > 5 points

  2. Definition - defines the components of each measurement

  3. Notes - add notes to describe important information.

  1. Point value scoring options for each measurement are: 1 points > 5 points.
    Score the assessment according to the appropriate criteria for the client.
  2. As measurements are scored, the score appears in the assessment grid next to the measurement.

Master Scoring Form

  1. Click the Scores button for the Transitions section or click the Calculator icon for each level.
    See Chapter 7 in the VB-MAPP Guide to learn more about the master scoring form.
  2. The Master Scoring Form opens.
  3. Information on the form: Assessment number, Date entered, current score, age of client (on testing date), color coding option, Assessor.

    NOTE: Use the color-coding option to assign a specific color to the assessment score.

  4. The scoring grid automatically fills in as the assessment is scored.
