The Data Collection team can make Custom Progress Reports/Notes for your organization. Custom Progress Reports/Notes are tailored specifically for your organization and can include many different types of information and data for learners and/or staff. This information can be represented by combinations of editable graphs, tabular reports, and anecdotal notes. Most organizations use them as a hybrid for capturing clinical and diagnostic information for reporting purposes.

Once our team has created the Report (Note), it is a template that can be used as often as you want at no additional charge.

Custom Template Requirements

In order to streamline the template-building process, we ask that you consider the following items when making a request;

  • Please submit your template in an MS Word document. This will need to be a rough skeleton of the Progress Note drafted as you wish to see it.
  • Do not include any PHI in this document, please. Instead use markers such as <Student Age>, <Student First Name>, <Student Last Name>, etc. 
  • Highlight in green all data that should be automatically populated from Catalyst (such as the markers above).
  • If you need to make any additional notes to our team, please do so using the "Comment Boxes" feature of Microsoft Word's "Review" functionality.
  • Be sure to translate items into Catalyst terms (e.g. Skill Areas, Target, Programs, etc.).

If you need a Custom Student Field, you would first create this yourself. See Create a Custom Student Field for more information. Then you would let us know the name of that custom field that you want to be pulled to the report. 

For sections that require formal assessments such as the Vineland, VB-MAPP, there are two options you can choose from:

  • Create a placeholder that clinicians can copy & paste the table or chart into.
  • Build out a table/chart from scratch & add prompts for clinicians to enter the needed information.

The report template will pull all the information listed in the reporting period based on the date range the user enters.
