  1. Label Name- Enter a name for the field (This will display on the UDF Tab in Patient Details).
  2. Control Type- Select the type of field.
    • CheckBox- Select to create this field as a check box.
    • ComboBox- Select to create this field as a combo box.
    • Date- Select to create this field as a date field.
    • Integer- Select to create this field as an integer field.
    • Real- Select to create this field as a real field (numeric values, allows decimals).
    • Text- Select to create this field as a text field.
  3. Sort Order- Select the order the field will show in.
  4. Max Length- Select the maximum number of characters that can be used in this field.

  5. Lookup Table- Click to add choices for Combo Boxes (only used when the Control Type is ComboBox).
