Lookup Manager Table of Contents

NOTE: Some Lookup Tables do not contain the Scoring Value as they were created prior to the development of Scoring. The example below is a table containing the Scoring Value. If the table you are adding fields to does not contain the Scoring Value, ignore step 4.

  1. Select the desired Lookup Table from the list.
  2. Click in the blank space of the grid of created lookup codes to remove any highlight.
  3. Name/Description - Enter the description of the field you want to add the lookup table.
  4. Scoring Editor - Assign the field a scoring value (associated with scoring in FormLabTM).
  5. Save Item - Select Save Item to save the new field added to the Lookup Table.

Repeat steps 2-5 to add additional fields.
