PERMISSIONS: My Profile is accessed by the user logged into the system.

 

My Profile contains your personal information and access to settings that you will use to personalize how you work. You can also change your electronic signature on this page.

Additional tabs are also available when you access your profile and include:

Access My Profile

To access My Profile:

  1. Click your name at the top right of the page.
  2. Click My Profile and settings.
  3. The My Profile page is displayed showing the additional tabs.

Profile Details

  1. The My Profile tab contains:

    1. Basic information:
      • First, Last and Middle Name
      • Staff Title
      • Titles & Certification - PhD, LCSW,
      • Title & Name - Displays how your name, title and titles & certification are displayed
      • Hire/ Termination Date
      • Degree Level
      • Contractor Status
      • Exempt Status
      • Staff Language
      • Service Area Zip Code
    2. Contact Information:
      • Email - a required field
      • Mobile Phone Number
      • Office Phone Number
    3. Calendar color preferences
    4. Default Location - main office location you use when providing services. Select from the dropdown list. Contact your system administrator if your location isn't listed.
    5. Default Note Type - when using custom, Dynamic Progress Notes you may select your preferred Note form here.
    6. Tax and Provider Information:
      • Tax ID - you may use your SSN or EIN.
        Required if you a Billing Provider when submitting claims.
      • Provider NPI
        Required if you a Billing Provider when submitting claims.
      • Provider License #
      • DEA Number
    7. Change my electronic signature - Your electronic signature can be created initially when you first sign a document. This link allows you to create a new signature here, change it or upload an image of your actual signature.
