Save a Completed Intake Form to the Patient

Permissions Required: You can only see intake forms for patients you have access to.

 

When a completed intake form is submitted by a portal user, you can save it to the patient's files. These forms have a Submitted date to indicate they've been returned to you.
 

Save a Completed Intake Form
 

  1. Go to Schedule → Pending Intake Forms.
  2. Click the form to view it.
  3. Review the form to be sure it has all of the needed information.
  4. Click the Save to Patient button.
     

The form will appear in the Files → Completed Intake Forms section of the patient's information.
