MN CCBHC Reporting: Screening for Social Drivers of Health (SDOH) Measure

Overview

The Screening for Social Drivers of Health (SDOH) measure evaluates screening for social factors affecting health. 

Measurement Parameters

Measurement Year (MY): January 1 - December 31 (12-month calendar year).

Eligibility Criteria

To be included in the SDOH screening measure, individuals must meet all of the following conditions:

1. Age Requirement
   • Must be 18 years or older at the time of screening.
2. Provider Visit Requirement
   • The client must have been seen at least once by a provider during the measurement year.
3. Screening Requirement
   • The SDOH screening form (PRO-1641) must be completed at least once within the same measurement year.
4. Scoring Note
   • The form will be scored; however, the score itself is not used to determine inclusion in the measure.

Data Entry

Data can be entered within the Client or Payer tab. Data is arranged by the following categories:

• Ethnicity
• Racial Groups
