Overview

The Adolescent Depression Screening and Follow-Up Plan (CDF-CH) Measure evaluates the percentage of clients aged 12 to 17 who were:

  1. Screened for depression on the date of an outpatient encounter or within the 14 days prior, using an age-appropriate standardized tool.
  2. Documented with a follow-up plan on the date of the eligible encounter if the screening result was positive.

 

Measurement Period

Measurement Year (MY): January 1 - December 31 (standard 12-month calendar year)

 

Eligible Population

Clients must meet all of the following criteria:

  • Age 12-17 on the date of the outpatient encounter.
  • Had at least one outpatient visit during the Measurement Year.

 

Data Entry

Enter data on the Client or Payer tab.
Data can be arranged by:

  • Payer
  • Ethnicity
  • Racial Group

 

Numerator Criteria

Clients who meet the following:

  • Depression screening performed on the date of the encounter or within 14 days prior using one of the following tools:
    • PHQ-9 Forms: PRO-0035, PRO-1592, K100550, or K100551
  • AND, if the screening is positive, a follow-up plan is documented on the same date as the eligible encounter using one of these codes:
    • G8431 - Positive screen with follow-up plan documented.
    • G8510 - Negative screen (no follow-up needed).

 

Exclusions

Clients are excluded from the measure if they have a diagnosis of depression or bipolar disorder, identified by:

  • Code G9717 OR a list of ICD-10 diagnosis codes for depression or bipolar disorder.

 

Exceptions

Clients who do not meet the numerator criteria but meet the following exception should be removed from the denominator:

  • Code G8433 - Screening for depression not completed due to documented patient or medical reason.

    NOTE: If a client meets the numerator criteria, they are included in the denominator regardless of exception status.
