To set up Patient Designation for CFTSS services, complete the following:

1. Navigate to Patient Details for the desired patient (3 Ways).
   1. Go to Day to Day 🡪 Patient Details 🡪 Search for the Patient.
      
      
       
   2. Right-click on the specific patient’s name from My Client List and select Patient Details.
      
      
       
   3. Select Patient’s name from top left corner of their chart.
      
      
2. Once you have selected the desired patient, select the Patient Designation Tab.
3. Health Home Information- If a client is enrolled in a Health Home, the Health Home information can be entered here.
4. Save and Repeat these steps for each patient that will be receiving a designation(s).
