  • Associate appropriate CPT/Procedure codes.
  • Associate appropriate form (Progress Note).
  • Associate appropriate Session Type.
  • Associate appropriate Visit Type Min / Max.
  • Associate appropriate Program-specific entry.
  • Exclude from TP, if applicable.
  • Adult BH HCBS Assessment / ISP, if applicable.

We (TGI) provided the (below) new HCBS Forms:

• HCBS_IPN- Adult BH HCBS Individual Progress Note
• HCBS_GN- Adult BH HCBS Group Note
• HCBS_DP- Adult BH HCBS Approved / Authorized Final Visit
• HCBS_AA- Adult BH HCBS Assessment
• HCBS_ISP- Adult BH HCBS Individual Service Plan
• HCBS_PCAR- Adult BH HCBS Prior and Continuing Authorization Request Form
• HCBS_ASR- Adult BH HCBS Additional Service Request or Extended Service Request

• Add R69, Illness, unspecified.
• Create Transportation types:
  • Round Trip
  • Per Mile
• Create a generic Assessment Instrument.
