1. Select the Insurance. Click in the box and press enter.
   • A drop-down list will appear.
2. Under Procedure, select the HCBS Service associated with the current chart being filled out.
   • If the patient has more than one chart and service, multiple services can be entered.
3. Enter Starting Units.
4. Enter Remaining Units.
5. Enter Max Units Daily.
6. Enter Max Units Weekly.
7. Enter Max Units Monthly.
8. Enter Start Date of services as provided by Health Home.
9. Enter Expiration Date as provided by Health Home.
