1. Select the appropriate service plan type, i.e. New, Continued (Partially Met), etc.
2. Select the Need to be developed from the drop-down menu.
   • This is populated from the saved Assessment (HCBS_AA).
3. Enter a number for the Goal.
4. Type in the Goal Statement.
   • This should be the same as the Need.
5. Select the Target Completion Date based on your analysis.
6. Based on your analysis, fill in as needed.
7. Based on your analysis, fill in as needed.
8. Click Add/Update.
