Patient Registration Part 1:
- Enter first and last name of patient.
- Enter the patient's date of birth.
- Enter the patient's sex.
- Enter the patient's address.
- Enter the patient's city, state, and zip code.
- Auto Generate medical record No can be set to random or sequential.
- Select the Save button.
- Payer will auto-fill with Responsible Party
- If you are adding an insurance, select the check box Same As Patient in the row.
- Relation will auto-fill.
- Enter Patient Insurance in Payer box.
- Enter Med ID of patient.
- Click Save.
Patient Registration Part 2:
- Enter Health Home Name.
- Enter Care Manager Name.
- Enter Care Manager Phone number.
- The Health Home table allows you to save important HCBS information related to the patient.
- Select the appropriate Tier from the drop-down list.
- The date will prefill with the current date. Change if necessary.
- The Adult BH HCBS table allows you to identify what tier of service the patient is receiving.
- Click Save.
- Click New Chart.
Patient Registration Part 3”
The User will prefill with your information. Change if necessary. The initial user being added will be the primary on the chart. Multiple users can be added to the chart.
- Click Add.
- After the user is added, a blue row will appear in the Users table as shown below.
- After the user is added, a blue row will appear in the Users table as shown below.
- Select the HCBS Program from the drop-down list.
- Select the Location from the drop-down list.
- Click Save/Open Chart.
Patient Registration Part 4:
- The arrows on the chart allow the end-user to move the patient from Referral to Intake to Admitted to Discharged.
- Chart status can be moved in either direction.
- Click the blue arrow to move the patient to Intake.
- Select Intake Date.
- Click Save/Close.
NOTE: While in Referral or Intake status, the red down arrow is available to Do Not Admit the patient.
