Home Tab
Upon login, the patient will land on the Home tab, highlighting the features of the Patient Portal.
Form Portal Tab
The Form Portal is a section of the Patient Portal where a patient can complete Form Portal forms designed and built within the FormLabTM in eCR. This tab will only be visible if there is an available Form Lab form for the patient.
IMPORTANT ITEMS TO NOTE:
- Form Portal forms only allow for specific FormLabTM controls to be used. Images of the FormLabTM controls and the Patient Portal view are below.
- A Form Portal form will automatically be accessible in the Patient Portal once access to the Portal is initiated and there is an active chart that includes a Form Portal form.
- A Form Portal form can only be completed once. Once completed, the form will no longer be accessible in the Form Portal.
- Only the patient (client) signature is available in the Patient Portal.
- Currently, no alerts are generated when a form has been completed.
Once a Form Portal form has been completed, you will see the completed form in the patient's chart.
Note that there are no user initials or a name next to the form version because this was completed by the patient.
Patient Medical Information Tab
The Patient Medical Information section of the Patient Portal allows a patient to view any CCDA files queued for this individual.
A CCDA, or Consolidated Clinical Document Architecture, is a file that contains numerous components of the patient’s episode of care such as demographic, allergies, medications, diagnoses, and services provided.
To queue a CCDA for your patient, go to the patient’s Patient Details > Additional Demographic Info (cont) tab. In the bottom left-hand corner, click the Create CCDA button. In the secondary window that opens, the first 2 options are queueable in the Patient Portal.
Appointments Tab
In the Appointments tab of the Patient Portal, the patient can see and keep track of appointments.
In the event the patient has no appointments scheduled, they will be greeted with this message.
In the event the patient does have appointment(s) scheduled, they will see a calendar with the appointment listed on that day and organized by time. The patient can change the view between a day, week, or month.
By clicking on the appointment, the patient will be provided with all the details the user has entered in the Schedule.
Secure Messaging Tab
Within the Patient Portal, patients have the ability to have direct communication with their provider or primary clinician through Secure Messaging. Secure Messaging is similar to sending a basic email. However, an important item to note, only a user in the system can initiate the conversation.
eCR User Messaging
As an eCR user, you may begin a conversation with a patient by following these steps.
- Go to Patient Details > Patient Portal Admin tab.
- Click the Secure Messaging button.
- Here you can see your messaging history as well as create and manage messages.

To send a message, click Create New Message.

Fill in the relevant information. The subject and the body of the message are required.

Click Send.
Patient Secure Messaging
In the Secure Messaging tab, a patient can view the messages sent by their provider or other eCR users.
The patient can reply by clicking on the message.
Important items to note!
- A conversation can only be started by a user in eCR. The patient cannot initiate a message chain.
- Presently, no alerts are generated for either the patient or eCR user when a message is sent or received.
- This functionality can either be turned on or off by the Development Team.
Direct Status Tab
This is an additional paid feature. For more information, submit a Helpdesk ticket.
Support Tab
The Support tab contains a few items that are not commonly used by the patient but can assist with troubleshooting if needed.
Audit Log
The Audit Log shows the patient what activities they have performed in the Patient Portal. It is filterable by date.
Help
In the event the patient needs assistance in the Portal, they will be directed to contact their provider as the system would not be able to assist your patient.
Server Time Check
The Server Time Check displays the Current Server Time and Version of the Patient Portal.
My Account
In the My Account section of the Portal, a patient a reset their password and update the email address associated with their account.
The patient's email is initially populated with what is populated in the patient's Patient Details > Name/Address tab > Email field within eCR. Once the Portal has been initiated, the email field in eCR is no longer connected to the Portal. If the patient updates their email address in the Portal, the change will not be reflected in eCR.
Forgot Password
If your patient forgets their password for the Patient Portal and clicks the Forgot Username/Password link on the login page, they will be direct back to their provider to have their password reset.
If the patient tries to reset their password prior to logging into the Portal, they will be instructed to reach out to their healthcare provider.
Once the patient has logged into the Patient Portal at least once, they will be able to reset their password using the Reset Password link on the login page.
- They will be asked to enter their Patient Portal ID.
- This was provided to them in the initial setup instructions. If they no longer have this number, they will need to reach out to their healthcare provider to reset their password.
- Click Submit.
- They will be asked for their Patient Username.
- Click Submit.
- The patient will enter the new password twice.
- ClickÂSubmit.
- If entered correctly, the patient will see a confirmation message.
- The patient can now log in with their new password.
Generate a One Time Use Password for a Patient

Go to Patient Details > Patient Portal Admin tab.

You will see a large green Generate On-Time Use Patient Password button. Upon clicking this button, this will delete their current password and will require the patient to use the generated password to log in.

After clicking the button, the one-time-use password will display. The password and instructions can be provided to the patient.

When the patient uses this one-time password, they will be immediately prompted to update their password.
