Family Setup: SA021 Service Activity Log Form

Once a patient is set up in the Family Setup and the data is saved the patient’s Relation and Family will pre-fill in a new SA021 Service Activity Log.

1. Open the patient’s active chart structure.
   
   
    
2. Open a new SA021 Service Activity Log.
   
   
    
3. The Relation and Family fields are pre-filled with the Relationship to Head and Family Name that was set up in Family Setup.
