Family Setup: Family Service Activity Report

Patients only appear if they have a signed SA021 Service Activity Log done within the filter Date Range.

  1. Go to Reporting 🡪 Reports.

  2. In the search bar enter “Family” and hit enter.

  3. Select the Family Service Activity report.

  4. DOS - Date Range- Select the date range that corresponds with the signed and completed SA021 Service Activity Logs.

  5. Family Name Filter- Remove the flag for NULL and type the family name you want to search for.

  6. Choose Family- Select the family from the drop-down list.

  7. Patient Name Filter- Remove the flag for NULL and type the patient name you want to search for.

  8. Choose Patient- Select the patient from the drop-down list.

  9. View Report- Edit any additional filters and click the view report button to run the report.
