Patients only appear if they have a signed SA021 Service Activity Log done within the filter Date Range.
- Go to Reporting 🡪 Reports.
 - In the search bar enter “Family” and hit enter.
 - Select the Family Service Activity report.
 - DOS - Date Range- Select the date range that corresponds with the signed and completed SA021 Service Activity Logs.
 - Family Name Filter- Remove the flag for NULL and type the family name you want to search for.
 - Choose Family- Select the family from the drop-down list.
 - Patient Name Filter- Remove the flag for NULL and type the patient name you want to search for.
 - Choose Patient- Select the patient from the drop-down list.
 - View Report- Edit any additional filters and click the view report button to run the report.
