PERMISSIONS REQUIRED: Clinic Staff must have Administrator, Billing Assistant, Billing Manager, Clinical Director, Front Office, Operations Manager, PTA, Staff Therapist, or Therapy Student assigned in order to manage some of the features in Charges & Documentation.
We understand that managing all of your cases is a critical function of any practice. We have created the system to not only be easy to use, but also allow any therapist to be able to sit down and with very little instruction be able to complete an evaluation or a daily note. We have designed the actual documentation to be as customizable as possible. This series of introductory articles will cover the essential aspects of the Charges & Documentation.
Diagnoses Panel
The next panel is the Diagnoses panel which contains all of the currently associated diagnoses.

| L/S | Level and Source | Level may be Primary or Secondary, Source may be Therapist or Doctor and are abbreviated P, S,T, and D respectively. |
| Description | Description of the ICD9 code. |
| Active | Displays the status of the diagnosis. |
This panel works identically to the Diagnoses panel in the case dialog and allows you to add, edit, and delete diagnoses.
NOTE: If all diagnoses are deleted and the Add Last Charge Set function is used, this will result in no diagnoses being linked to a charge.
