Permissions: You must have Billing Permissions.
To generate a CMS 1500 form you must create an invoice first. You can print a blank CMS1500 Form from the Client Ledger.
- Locate Client and open Client Profile.
- Click Ledger in side menu.
- Click Actions and select View & Print Blank CMS 1500 for appropriate Invoice.
