  1. A completed PHQ-9 in the patient’s chart will prefill previous session info into the PHQ-9/Questions page.
• Fill to Logic – Yes
  1. The Score and Answers from the PHQ-9 imbedded into the form will fill the Tracking Charts. The score and answers will prefill into the Session 8 section of the Tracking Chart Form. Once the information is pre-filled, you can edit or fill in missing information on the form.
• Signatures
  1. Staff Signature
     • Credential required to sign: any sig type.
     • Title will fill from title entered in user setup (see eCR University – Users).
     • Signature required.
     • Route To functionality is available.

Questions Tab

A completed PHQ-9 in the patient’s chart will prefill previous session info into the PHQ-9/Questions page.

1. Select / change appropriate options/ scores.
2. Total score will calculate based off of the options selected.
