Session Info Tab

1. Organization Name and Program Name will populate for Billing/Program Defaults.
2. Select appropriate Date of Service.
3. Enter appropriate Start and Stop Time for the service.
4. Select the Service Code (Visit Type) which populates from the list of Visit Types created in the system.
5. Individuals Present - select appropriate options.
6. Prescriber’s Evaluation - Complete section be entering in the necessary description and/or selecting the appropriate options.

Mini-Mental Status Tab

1. Complete Mini-Mental Status. If this is not necessary select N/A making the tab not required.

Goals/Objectives Tabs

1. Complete section by selecting appropriate options and entering in necessary descriptions.
2. Select Goals and Objectives being addressed in the session. Capable of addressing up to 4 Goals and 4 Objectives for each Goal.

Med Renewal/Changes Tab

   • Route to Function is available.
   • Signature required to lock form.
