The vital signs grid can be added to client forms, so that vitals entered in the client form will pull into patient details, and vice versa. This means you can document vitals while in a client form, without having to navigate back to the patient details screen.

Add Patient Vitals to a Client Form
- Click Administration from the menu bar at the top.
- Select FormLab.
- Click Start New Form.
- Enter Form Name, Description, Tab Label and Special Label.
NOTE: Patient vitals can be added to all form types except the Form Portal Form.
- Click Ok. The Client Form Designer window opens.
- Right-click on Section.
- Hover over Add Public Health Control, then select Patient Vital Signs.
- Enter a Patient Vital Signs Label.
- Click Ok.
- Right-Click on Section again as-needed to add other Controls to the form.
- Click Preview to review the form before it is published.
- Click Save Form Design.
- Click Publish.
Add Patient Vitals Form to a Program
- Click Administration from the menu bar at the top.
- Select Form Manager.
- Use the Program Filter to search for a program.
- Click on the Program. The Chart Structure Designer opens.
- Enter the name of the form you just created in the Search bar under the Forms List.
- Click on the form in the Form List.
- Drag and drop the form into the current chart structure list.
- Click Set Active.
- Close out of the window.
Enable Form Access for User

Document Patient Vitals in Client Form
- Open the form for the patient.
- Click inside the Column Fields to enter vitals data.
- Click Save.
- Click Staff Signature and enter signature to lock the form.
- Vitals Data that was entered in the client form displays in the Vital Signs grid in Patient Details.
NOTE: Vitals data that was entered in a client form must be edited in the client form. Vitals data entered in a client form displays in yellow and is read-only in Patient Details.
