• Pre-Fill Logic
  1. Diagnosis will prefill from locked and saved Diagnosis Form in same program.
  2. Diagnosis will prefill from an Assessment or any form that included a diagnosis.
• Fill to Logic
  1. Diagnosis will fill the read-only Diagnosis in the Diagnosis Tab in Patient Details.
  2. The diagnoses entered will fill the diagnoses in the Diagnosis Tab in an Assessment or any other form that includes the diagnosis control in the same program. Once the information is pre-filled, you can edit or fill in missing information on the form. This will also fill to the read-only Diagnosis in the Diagnosis Tab in Patient Details as well as a new Diagnosis form.

     • Signature locks the form

Diagnosis

1. Enter the diagnosis for the patient. Diagnosis list will populate from a list of diagnoses entered enter the system.
2. Select Primary, if the diagnosis entered is to be the primary diagnosis for the patient.
3. Select Specifier if needed.
   
   
    
4. Enter any additional comments for the diagnosis as needed.
5. Date diagnosed will populate to the date in which the diagnosis was entered.
6. Date modified will populate if the diagnosis is modified after the date diagnosed.
7. Select if diagnosis is resolved.
8. Select PoA if needed.

   • Route To is available
   • Signature locks the form
