Summary
Consent for Release is a form that an agency provides to a patient that gives an agency permission to share the patient’s information with a predetermined person and/or organization. You can create as many Consent templates as necessary. These templates include the Extent and Purpose of the Patient’s Information to be disclosed and to whom.
