Once in Form Manager, select Setup Release. Setup Release allows you to create Consent for Release templates that will prefill data into Consent for Release forms on a patient’s chart. You can create as many templates as you would like. Follow the steps below to design the Consent Template.
If you are editing an existing template, you can search for and modify an existing Consent for Release Template using the Search for Report.
- Click Enter while your cursor is in the text box to pull up the list of created templates.
- To create a new template, fill out the information in the Details 1 and Details 2 tabs and click Save.
