Clinical Overview: Create a Patient's Chart

1. Click the Save/New Chart button (This will allow you to assign a primary clinician and location to be able to schedule an appointment).
   
   
    
2. The clinician’s name will appear in the User: field.
3. Select the Update button to the right of the comment field (Your name will now appear “checked” at the top of the users list below as the Primary User).
4. Select a Program, Location, and Referral Date.

   NOTE: The Program and Location will default to the Program and Location of the user.

5. Save/Open Chart- Click this button to save all entered information and open the chart.
