This article outlines the process of requesting a set of new clinical documents. New sets are needed for new customers and when existing customers add new programs.

Decision Stage

Prior to your project consultation, complete the following:

  1. If you are interested in reviewing your Clinical Documents, ask your Implementation manager about the clinical document sandbox.
  2. Provide a list of all clinical documents needed, also known as your project request.
    • Include the following:
      • Priority - what needs to be completed first, second, third, etc.
      • Form number and name
        • Example: SHN-1113 Alternate Diagnostic Assessment
      • Form type - Diagnostic Assessment, Treatment Plan, Progress Note, Discharge, Client, etc.
    • How to upload a project request:
      1. Create a Support Ticket

      2. Attach A File to A Ticket

  3. Review the list of Agile Process Questions

NOTE: Based on the number, size, and complexity of your documents, we may suggest that you allow us to build your documents in multiple phases called “Sprints”. That will allow us to deliver your most critical documents quicker.
