Permissions Required: You need to be the primary therapist or co-signer for a document to revise it.

 

If you need to make changes to a document that's already been signed, you can revise the document. If goals have been achieved or data points for the date of service have been changed, the document will be updated to reflect those changes. There's no way to undo those changes once the document has been revised. Revising a document removes all signatures from it. A revised document will appear as a draft in the primary therapist's Docs tab.

 

If your clinic's Document Revision History option is set to Detailed, the therapist will be asked to provide a reason for the revision when re-signing the document. This list of revision reasons can be printed or included when sending the document.

 

Revise a Document
 

  1. In the upper left of your screen, click inside the Search Patients field to search for and select a patient.
  2. In the lower left, click the Documents button.
  3. To the right of the document, click the Revise Document icon revise_icon.png.
  4. Click the Revise Document button.
