Configurable Evaluation Sections

Configurable sections within the Fusion evaluation template can be added to various documents including Initial Evaluations, Progress Notes, Re-Assessments, and Discharge Summary documents, specifically in the Performance Skills/Findings and Areas of Occupation/Functional Measures steps.

 

This feature allows you to collect data that is pertinent to the fields your clinic specializes in and meet your individual needs.

 

Fusion will work with you to build these configurable sections into your clinic documents. To get started, please contact Support to request a quote using the Community Portal.

Text Field

 

The Text Field area allows for a small amount of text in your evaluation form.  See our example below. You can use this section for:

  • Short descriptions
  • Basic information

Text Area

 

The Text Area section allows for a larger amount of text in your evaluation form than the Text Field.  See our example below. You can use this section for:

  • Larger descriptions
  • Various observations

Drop Down

 

The Drop Down area gives the option to select a fixed set of selections in your evaluation form.  See our example below. You can use this section for:

  • Options to select from
  • Controlled selections

Radio Buttons
 

The Radio Buttons area gives the option to select a fixed set of selections in your evaluation form.  With Radio Buttons, you can only select one and not multiple choices.  See our example below.  You can use this section for:

  • Preset options
  • Yes/No Questions

Checkboxes

 

The Checkboxes allow you to select a fixed set of selections in your evaluation form.  With Checkboxes, you are able to select multiple choices.  See our example below.  Examples of how you could use this section:

  • Yes/No Questions
  • Select symptoms
  • Level of Impairment

Table
 

The Table section allows for the creation of a configurable table with each of the following fields (text, radio button, checkboxes, date, pictures).  With Tables, you are able to group multiple fields in the same table.  See our example below.  Examples of how you could use this section:

  • Responses to common tests/assessments
  • Items that have multiple responses
  • Grouped information

Picture

 

The Picture section allows for the add a picture.  There is a maximum size of 4 MB. You can also select to have the ability to annotate the picture.  See our example below.  Examples of how you could use this section:

  • Use as body pictures within an evaluation.  You are able to note specific information at a given point on the picture.

Paragraph

 

The Paragraph section allows for an area of longer text input.  With a Paragraph section, you are able to select a narrow or wide format for the section.  See our example below.

Examples of how you could use this section:

  • Use this for any further explanations that may be needed.
  • Longer more detailed entries
  • Miscellaneous information

NOTE:  These are fixed paragraphs only.  This means they cannot be edited per evaluation and will be uniform across all of the evaluations where the section is added.

 

Date

 

The Date section allows the ability to enter a date via a calendar selection rather than inputting the date via text entry.  See our example below. 

Examples of how you could use this section:

  • Date of the last service
  • Date of injury
  • Any specific date of importance.
