Loc ID: Unique location ID passed from the EHR to the system; one subaccount may have multiple locations - this is how the system tells them apart.

Location: The location name assigned to that physical address & Loc ID.

Last: Provider last name.

First: Provider first name.

NOTE: Provider first name and provider last name must match NPPES, so if there is a middle name, etc. or the name has been changed, this will need to be updated to reflect DEA records for EPCS.
DEA: DEA Number. If a provider is prescribing controlled substances for multiple states, they will need to connect to the system at each location to create a new user row and register with that unique address in order to prescribe out of state.

NPI: National Provider Identifier, 10 digit-long unique identification number for covered health care providers; we use the NPPES database to confirm provider name matches what is on file with Surescripts.

Doc ID: Unique individual identification tag for this provider; if they have more than one prescriber account, for example, they have both a Midlevel & Doctor level role. The DocID should be the same across locations, not across roles.

D/M: Are they a Midlevel (M) or a Doctor role (D)? Doctors can prescribe without a supervisor; Midlevels need to select a supervisor in order to prescribe.

SS: Are they registered for Surescripts? If not, this will be blank. If there were errors, it will be yellow and say “queued”; if it’s successful (010 Verified/Registration Successful!) it will either be Green if they don’t have renewals, or blue if they do.

EPCS: If this is blank, the prescriber cannot send controlled substances / is not registered for EPCS; if it filled in, that date is when the account first granted & finalized the provider and first registered them for EPCS.


NOTE: If this says Grant, but does not say Final, the provider has not completed the EPCS registration process and will not be able to send controlled substances until they do.
IDP: Has the provider been identify proofed in the system? (This is not always checked; if it is not checked, it should not be an issue) (If the provider is EPCS registered, Exostar’s identity proofing is sufficient.)
