It is a user-friendly report view of an adolescent's PHQ-9 scores over a specified period of time. The report parameters include a date range and the client code. A line chart of the scores over time displays beneath the results table for more visual analysis.
1. The report parameters are required:
- Start Date
- End Date
- Client Code
2. The report Header displays the parameter information entered.
3. The results are displayed in ascending date order. These include:
- Assessed Date
- Administered By
- Total Score
- Depression Severity - A value must be selected in this field on the PHQ-9 Adolescent assessment for the record to display on the report.
4. The line chart is a graphical display of the table results.
