FormLab: Create a Treatment Plan

When creating a Treatment Plan in FormLabTM, it is required that the Make Treatment Plan option be selected under Special Form Types. This option will load in the necessary fields to make the form a Treatment Plan. The Goals/Objectives loaded will pull the needs from the Assessment and “talk” to the Billable Progress Note. There must be at least one signature control that is labeled Treatment Plan Completed. See below for the steps required to create a Treatment Plan in FormLabTM.

  1. Go to Administration 🡪 FormLabTM.
  2. Click Start New Form.
  3. Enter a Form Name.
  4. Under Special Form Types, select Make Treatment Plan.
  5. Enter a Tab Label and Section Label.
    • Check the box to display borders or a label, if desired.
  6. Click Ok to load the initial form design.

  7. This will pre-load the minimum required fields for the Treatment Plan. You can add any additional desired tabs, sections and controls. You can add controls to the Goals and/or Objectives Tabs but there are limited options available.
  8. To create a Signatures tab:
    • Right-click on [Form].
    • Click Add Tab.

    • Enter Signatures in the Tab Label field.
    • Enter Signatures in the Section Label field.
      • Check the box to display borders or a label, if desired.
    • Click Ok to add the new tab to the designer.

  9. To add a Signature Control to the Signature Tab/Section:
    • Right-click on [Section].
    • Select Add Control.
    • Select Signature.

    • In the Signature Label field, enter a label for the signature.
    • For Signatory, select User Signature.
    • From the Signature Type drop-down menu, select the signature type.
    • Under General Options, select Show Route To, Required Field and Treatment Plan Completed.
    • Click OK and the control will appear under the associated section.

    • You may add additional signatures to this section, if desired.
  10. Click Save Form Design.
  11. Click Preview to see what the published form will look like.

  12. Click the Publish button once you are satisfied with your form.
  13. The form has now been added to the Form List so it can be added to a Program’s Chart Structure.
