When creating a Billable Progress Note in FormLabTM, it is required that the Make Form Billable option be selected under Special Form Types. This option will load in the necessary fields to make the form billable. For the Billable Progress Note to pull the Goals/Objectives from the Treatment Plan the Goals/Objectives Control must be added. Since the form is billable, there are additional Billing Controls available to add to the form. See below for the steps required to create a Billable Progress Note in FormLabTM.
- Go to Administration 🡪 FormLabTM.
- Click Start New Form.
- Enter a Form Name.
- Under Special Form Types, select Make Form Billable.
- Enter a Tab Label and Section Label.
- Check the box to display borders or a label, if desired.
- Click Ok to load the initial form design.
 - This will pre-load the minimum required fields to make the form billable. In order to make the Progress Note pull from the Goals/Objectives created in the Treatment Plan, you must add the Goals/Objectives Control.
- Right-click on [Section].
- Click Add Control.
- Click Goals/Objectives.
 - Enter the Goal Label for the goal/objectives control.
- Enter the Number Of Objectives (0-10) available to be selected.
- When the box for Required Field is checked, the goal will be required in the form.
- Click OK and the control will appear under the associated section.
 - You may add additional Goals/Objectives drop-downs by repeating these steps.
- You do not need to make any additional goals required.
- Add any additional desired tabs, sections or controls.
- To create a Signatures tab:
- Right-click on [Form].
- Click Add Tab.
 - Enter Signatures in the Tab Label field.
- Enter Signatures in the Section Label field.
- Check the box to display borders or a label, if desired.
- Click Ok to add the new tab to the designer.
- To move the Rendering Signature to the Signature Tab/Section.
- Click on the Rendering Signature Control and drag it over the Signatures Section.
- Release and the Control should now appear in that Section.
 - You may add additional signatures if desired now to this Section.
- Click Save Form Design.
- Click Preview to see what the published form will look like.
 - Click the Publish button once you are satisfied with your form.
- The form has now been added to the Form List so it can be added to a Program’s Chart Structure.
