Associate Forms With Programs

Overview

You have the ability to associate forms to specific programs. This way, when you go to select a form for your client, you will only see forms for the program(s) that that client is associated with. This is helpful if your organization has many program-specific forms (eg. ARMHS vs CTSS, Mental Health vs. Chem Dep, etc.).
 

IMPORTANT: Once a single form is associated with a program, all forms should be associated with a program. Any form(s) not associated with at least one program will no longer be accessible from the Clinical/Charting > Add window until you click the "Show All" option.

 

 

Open the Edit Forms Window

  1. Click on the Clinical/Charting Module.
  2. Click the Other actions drop down.
    • If you do not see the Other actions dropdown, click the blue link in the upper right corner of the screen titled more (located under the Help button) to display additional options.

  3. Select Edit Forms.
    • A new window will appear with a list of all of your current forms on the left hand side.
    • If you do not see the option Edit Forms, you do not have permission. You will need to check with your Champion. 

 

Associate Forms to Programs in the Edit Forms Window

  1. Select the form you wish to associate with programs from the left side of the screen.
  2. Highlight the program(s) you wish for the form to be associated with on the right hand side of the screen.

    NOTE: In order to associate more than one program to a form, you will need to hold the Ctrl key (or on a Mac) on your keyboard and then make your selections. 


     
  3. Go through the list of forms and associate each with its respective program(s).
  4. Close the window. 
    • Your selections will automatically save.

 

Select Forms in the Clinical/Charting Add Window

Once you have associated forms with programs...

  1. Go to the Clinical/Charting Module. 
  2. Click Add.
  3. Select a client.
    • Here you will see the list of forms associated with the clients program(s).
  4. If you do not see the form you need to fill out for the client, click the Show All check box in the Clinical/Charting Add window (see below).
    • This will expand the list of forms to include all forms for your agency, even if they are not associated with a program.
