Guardian Verification Signatures for Parental Verification Form

When downloading the Parental Verification Form there are two ways to get the signature of the Parent or Legal Guardian to display for the client’s appointments.

  • If a client signature has been collected for the client on the progress note that is attached to the associated appointment on the form, then this signature will be displayed here. This can be helpful if you are using the Ensora Data Collection integration where parents/guardians sign off on notes.
  • You can enable the Guardian Appointment Verification Signatures feature to collect guardian signatures on the appointment information from the client’s appointment history page within the system.

Enable Guardian Appointment Verification Signatures Feature

This setting will allow you to obtain a signature from a parent/guardian to verify the details of an appointment so that it can be added to the Parental Verification Form. This signature is collected separately from the client signature on progress notes and will not lock progress notes. This can be collected from the client's appointment history page within the system from the start of the month that this setting was first enabled for any appointments that are not in the future. Enabling this setting will also allow you to collect a Monthly Guardian Verification signature that can be added to the last page of the Parental Verification Form.

  1. Go to Organizations & Settings.
  2. Click the Profile tab.
  3. Click checkbox for Enable guardian appointment verification signatures.
  4. Click Save Changes.

  5. Once the setting is enabled, you will see an additional action to Collect Guardian Verification Signatures from the Client’s appointment page for any appointments that are in the Upcoming or Kept status that are not in the future. There is also a new column on the Appointments page called Guardian Verification which will show statuses of Signed or Not Signed so you will know which appointments still needs a signature collected.

Collect Guardian Appointment Verification Signatures

  1. Go to Client’s Details page and Appointments or click the Reports tab > Appointments

  2. Find the appointment you want to collect the Guardian Verification signature for and click Sign Guardian Verification from the Actions Dropdown.

    NOTE: Only appointments that are in an Upcoming or Kept status that are NOT in the future and for the month that the “Enable guardian appointment verification signatures” setting was first enabled and going forward will have the ability to collect this signature.

  3. The Guardian Verification modal opens with the Client’s appointment information at the top.
  4. Enter in the name of the person who is verifying the appointment.
  5. Click Draw signature.
  6. Use your mouse or finger to sign for the appointment and click Save. You can also clear your signature or cancel if needed.

  7. After collecting your signature, you will return back to the modal where you can see a copy of your signature. You can click on Edit signature if you need to change the signature. 
  8. Check the Legal Consent checkbox.
  9. Click Save.

  10. The Guardian Verification column in the Client’s Appointment page will be updated to the Signed Status and an event will be added to the Activity log when this signature has been collected. This signature will then also appear on the Parental Verification Form.

Edit Guardian Appointment Verification Signatures

After a Guardian appointment verification signature has been collected, you can edit this signature if needed.

  1. Go to Client’s Details page and Appointments or click the Reports tab > Appointments as explained above. 
  2. Find the appointment you want to edit the Guardian Verification signature for and click Update Guardian Verification from the Actions Dropdown.

  3. The Update Guardian Verification modal opens with the Client’s appointment information at the top. The previously collected name and signature will also show here.
  4. Click Edit signature.
  5. Use your mouse or finger to sign for the appointment and click Save. You can also clear your signature or cancel if needed. 
  6. Enter the name of the person that is verifying the appointment.
  7. Check the Legal consent checkbox.
  8. Click Save.

  9. An event will be added to the Activity log when this signature has been updated. This updated signature will also appear on the Parental Verification Form. See Parental Verification Form for more information.

 

Delete Guardian Appointment Verification Signatures

You can remove/delete a guardian verification signature for an appointment where a signature has already been collected.

  1. Go to Client’s Details page and Appointments or click the Reports tab > Appointments as explained above. 
  2. Find the appointment you want to delete the Guardian verification signature for and click Delete Guardian Verification from the Actions Dropdown.

  3. You will see a confirmation message: This action will delete the Guardian Verification Signature from this appointment. Would you like to continue?
  4. Click Continue to delete this signature or click Cancel to exit out.

  5. If you delete the guardian verification signature, this event will be recorded in the activity log and the signature will be removed from the Parental Verification Form for this appointment. The Guardian Verification column on the Appointment page will also update the status to Not signed unless there is also a client signature collected for the progress note that is attached to this appointment.

    NOTE: If you delete the Guardian Verification signature for the selected appointment, but the progress note linked to that appointment also contains a client signature, then the client signature will be displayed on the Parental Verification Form in its place. Both types of signatures need to be removed if you want there to be no signature image on this form.
