Import Clients to ShareNote Community

 PERMISSIONS REQUIRED: Supervisor or Administrator is required.

Supervisors or Administrators can import Client Members into the system. Using a Client Import Template you are able to enter basic Client information and then upload. Additional information can be added to the Client Profile after creation.

Download Client Template

  1. Hover over the Managers tab in the navigation bar and select Preferences from the drop-down.
  2. Expand Account Details section.
  3. Click Import Clients.

  4. Click Download Client Template and open the Client Template Excel document.

 

Fill In Template

  1. Open the downloaded Client Template Excel Document.
  2. Fill in columns for your Clients.

    NOTE: There are four mandatory fields - First and Last Name, Gender, and Primary Fund Source. The rest of the fields are optional.

    • First Name (Required)
    • Last Name (Required)
    • Middle Name
    • Program Start Date
    • Gender (Required) - M or F
    • DOB - Date of Birth
    • SSN - Social Security Number
    • Record ID# 
    • Weight
    • Height - Total Inches
    • Primary Fund Source (Required)
    • Policy Number - Client's Insurance Policy Number
    • Primary Dx - Enter the Primary Dx Code
    • Race
    • Address 
    • City
    • State
    • Zip 
    • Emergency Contact Name 
    • Contact Phone
  3. Once you have added all Client Information. Save the Template to your Computer.

 

Upload Client Template

IMPORTANT: Be sure you are not adding Duplicate Clients as they will be added in when uploaded. Since Clients could have the same name, the system does not filter Clients when uploaded. All Clients in the Import Template will upload. 

  1. Navigate back to the Account Details section of the Preferences page.
  2. Click Import Clients link.
  3. Click Choose File field in the Upload Client Template section.
  4. Select the Client Template you saved in the last section.
  5. Select appropriate Location if more than one Location exists for your Office.
  6. Click Upload Client Template.

  7. You will either receive a green confirmation box if the Import was successful OR a red failure box if the Import was unsuccessful.

  8. If your Import failed, fix the lines provided, save the Template again and re-upload.
