The South Oaks Gambling Screen (SOGS) is one of the Core Assessments included standard with the product. This form is a tool to assess the prevalence of pathological gambling.
The user can access the form directly in the Timeline by clicking the green + sign. The form window will pop-up displaying the assessment.
Client - The field is auto-populated with the selected Client.
Staff - The dropdown list is populated with Staff as defined in Configuration - Staff/Users > Staff.
Statements a-m each have the same available responses. A value is assigned to each response and used to calculate the Total Score. These responses are:
- Not at all
- Less than once a week
- Once a week or more
Statement 2. What is the largest amount of money you have ever gambled with on any one-day - The user can select from a set of available responses that are each assigned a value that is included in the Total Score. These responses are:
- Never Gambled
- $1.00 or less
- More than $1.00 up to $10.00
- More than $10.00 up to 100.00
- More than $100.00 up to $1,000
- More than $1,000 up to $10,000
- More than $10,000
Statement 3. Check which of the following people in your life has (or had) a gambling problem - The user can select any number of the available responses. Responses available to select are:
- Father
- Mother
- Brother/Sister
- My spouse/partner
- My child(ren)
- Another relative
- A Friend or someone important in my life
Statement 4. When you gamble, how often do you back another day to win back money you have lost? - The user can select from a set of available responses that are each assigned a value that is included in the Total Score. These responses are:

- Most of the time
- Some of the time (less than half of time I lose)
- Every time that I lose
Statement 5. Have you ever claimed to be winning money gambling, but weren't really? In fact you lost? - The user can select from a set of available responses that are each assigned a value that is included in the Total Score. These responses are:

Statements 16 a-k each have the same available responses. A value is assigned to each response and used to calculate the Total Score. Responses available to select are:

Total Score - Once responses have been selected for the statements, a total score for the SOGS is calculated and displayed in the field.
Remember to click Save to record entered information. A sample completed SOGS form is shown below.
The user can choose to sign the form by selecting the SIGN button. Another user can be notified that the SOGS was completed by selecting the SEND button. By selecting SEND and Review Requested, the message will show in their inbox with a link to the SOGS to be reviewed.
A completed SOGS form will also display on the Timeline. For this assessment, a green circle icon is the configured depiction.
