The PCL-5 with Criterion A assessment is one of the Core Assessments included standard with the product. This assessment is a tool to assess the 20 DSM-5 symptoms of PTSD with additional criteria met. The form used is the PCL-5 with Criterion A.
The user can access the form directly in the Timeline by clicking the green + sign. The form window will pop-up displaying the assessment.
Client - The field is auto-populated with the selected Client.
Staff - The dropdown list is populated with Staff as defined in Configuration - Staff/Users > Staff.
Briefly identify the worst event - The field is free-text and should be populated by the user.
How long ago did it happen? - The field is free-text and should be populated by the user.
Did it involve actual or threatened death, serious injury, or sexual violence? - The field has two available responses. Responses available to select are:
- Yes
- No
How did you experience it? - The user can select any number of the available responses. Responses available to select are:
- It happened to me directly
- I was repeatedly exposed to details about it as part of my job
- I witnessed it
- I learned about it happening to a close family member or close friend
- Other, please describe:
If the event involved the death of a close family or close friend, was it due to some kind of accident or violence, or due to natural causes? - The user can select any number of the available responses. Responses available to select are:
- Accident or violence
- Natural causes
- Not applicable
Questions 1-20 each have the same available responses. A value is assigned to each response and used to calculate the Total score. Responses available to select are:
- 0 Not at All
- 1 A little bit
- 2 Moderately
- 3 Quite a bit
- 4 Extremely
Total Score - Once responses have been selected for the questions, a total score for the PCL-5 with Criterion A is calculated and displayed in the field.
Remember to click Save to record entered information. A sample completed PCL-5 with Criterion A form is shown below.
The user can choose to sign the form by selecting the SIGN button. Another user can be notified that the PCL-5 with Criterion A was completed by selecting the SEND button. By selecting SEND and Review Requested, the message will show in their inbox with a link to the PCL-5 with Criterion A to be reviewed.
A completed PCL-5 with Criterion A form will also display on the Timeline. For this assessment, a green circle icon is the configured depiction.
