The Amendment Request form is a written request to amend or change a field on a previously completed form. User Group Permission to access must be granted in the VHR Timeline Category Depiction Manager prior to accessing the form from the Client's Timeline.

The Requestor may be designated as the Client or Other. A Request type of Other allows the user to enter the Other Requestor's name and title. The Form, Field Affected, and Amendment Requested are all free text fields. The Status of Request field may be used to indicate In Process (Blank), Denied, or Approved.

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

An Amendment History form is also made available so that previous request may be viewed in a list.
