Add or Update Client & Parent/Guardian Signature in Patient Portal

You can add your Signature in the Patient Portal. When you do you this, you will create a PIN which will be used as authorization when you want to apply your Signature to a document in the Client Portal.

Add Client Signature

  1. Navigate to https://myhealth.sharenote.com/
  2. On dashboard, click Create Signature.

  3. Add Signature using your mouse or touch pad.
  4. Create and Confirm your PIN.
  5. Select a Security Question.
  6. Enter and Confirm your Security Answer.
  7. Click Save Signature and Security Information.

Update Your Signature

  1. Navigate to https://myhealth.sharenote.com/
  2. On dashboard, click Update Signature.
  3. Enter PIN and click Click to Verify button.

  4. Follow steps above to update Signature.

Add Parent/Guardian Signature

If the Client is 18 years of age or younger, you can add the signature of a parent or guardian.

  1. From the dashboard, click Create Signature.

    NOTE: As soon as the Client is older than the age of 18, the option to add a Parent/Guardian Signature will disappear.

  2. Add Signature.
  3. Enter Parent's First and Last Name.
  4. Enter and confirm the PIN.
  5. Use the drop-down to Select Security Question.
  6. Enter and Confirm the Security Answer.
  7. Click Save Signature and Security Information.
