Delete a Dynamic Form
Dynamic forms can be deleted by users provided that no form instances have been signed or approved.
IMPORTANT: If data loss occurs due to form deletion, TheraNest by Ensora Health cannot restore or reinsert lost data into the system under any circumstances.
NOTE: You cannot delete any Dynamic Form that has been signed. The Delete button will be disabled and you will be instructed to archive the form instead.
The following message is displayed: “This form has been used and cannot be deleted. Please archive the form.

You MUST also always have at least one Dynamic Progress Note Form. If you try to delete the last Dynamic Progress Note Form in the list, you will find the Delete option disabled.
- Click the Dynamic Forms tab in the primary navigation bar.
To delete a form with existing client data:
- Click the Actions dropdown in the row for the specific form.
- Select Delete for the desired Dynamic Form.
- The Delete dynamic form modal will open.
- Review the warning message. It will display the name of all clients who have data tied to the form.
- Users can navigate to a client’s details directly from the modal by clicking the client’s name.
- Check the I understand that this action is permanent and cannot be undone, and any data collected through this form will be lost and unrecoverable. checkbox field.
- Click the blue Delete button.
To delete a form with no client data present:
- Click the Actions dropdown in the row for the specific form.
- Select Delete for the desired Dynamic Form.
- Click I understand.…
- Yes, DELETE EVERYTHING button to confirm.
