Add Dynamic Forms in the Case Notes Page

Permissions: You must have Therapist Permissions or higher.

You can create and add Dynamic Forms in the Client's Case Notes. It will display as a tab under Clients > Notes.

1. Click Dynamic Forms tab in primary navigation bar.
2. Click New Form button.
3. Select Case Form.
   
    
4. Customize rest of form as desired. See Create Dynamic Form to learn more.
5. Click Save Form button.
