Dynamic Forms is the way to build Client, Case, Organization, and Progress Note Forms. You can also add Dynamic Fields which pull information from the system into the Dynamic Form. 

NOTE: You cannot upload your own forms at this time. You must either create a dynamic form or edit existing forms.

 

IMPORTANT NOTES

  • Client, Case, and Organization dynamic forms can be distributed to clients using the Client Portal by copying the Dynamic Form to Intake Forms.
  • All of these forms can be distributed automatically as Intake Forms when a client is added to the client portal.

    NOTE: If a Case Form that includes the Client Diagnosis is in the default Intake Forms package, it will be included if there is no diagnosis. If there is a diagnosis the form will not be included in the intake form package.

  • Client, Case, and Organization dynamic forms can also be distributed from either the General Documents tab on Client Details or from the Clinical Documents tab on the Client Notes page.

    NOTE: If a Dynamic Case form contains the Client Diagnosis form element, it can only be distributed from the Clinical Documents tab on the Client Notes page.

  • If you are an existing client, these forms are accessible under the Archived tab:
    "No Secrets" Policy for Couple/Family, Audit-C, ACEs Questionnaire, CAGE Substance Abuse Screening Tool & DAST-10.

Client Form

Are placed in the Client Details page under the Client Details tabs. Used for general information about a client and available to anyone with permission to view client details.

Case Form

Used for general and clinical information about a case and available to anyone with therapist permissions or higher to view case details.

The tab can be accessed from the Client Details page and selecting Notes from the side navigation.

Dynamic Case forms may also include a Client Diagnosis form element. When the Client Diagnosis form element is included on the form it can only be distributed to clients using the Client Portal through the Clinical Documents tab.

Organization Form

Placed in the Dynamic Forms tab. Used for general/standard information for the organization and can include policies, procedures and other information that can be printed to be completed or added to Intake Forms and accessed and completed by clients in the Client Portal. Organization Forms do not use dynamic elements.

Progress Note Form

Used for client progress notes and are linked to clients and can be restricted to specific Therapists based on their access to the client. Dynamic fields and information included differ from the other dynamic forms. See Create a Dynamic Progress Note for details.

Progress Note Forms are chosen when a Progress Note is created on the client Notes page.

NOTE: SOAP Note and Standard Note forms are included in My Clients Plus by Ensora Health by default. Any Progress Note Form can be set as the default Progress Note by individual Therapists.
