Delete a Dynamic Form

NOTE: You cannot delete any Dynamic Form that has been used to collect information. The Delete button will be disabled, and you'll be instructed to archive the form instead.
You MUST also always have at least one Dynamic Progress Note Form. If you try to delete the last Dynamic Progress Note Form in the list, you will find the Delete option disabled.

 

1. Click the Dynamic Forms tab in the primary navigation bar.
2. Click Actions and select Delete for the desired Dynamic Form.
   
    
3. Click Yes, Delete it button to confirm.
