PERMISSIONS: You must have Therapist Permissions or higher.
You can create and add Dynamic Forms in the Client's Case Notes.
- Click Dynamic Forms tab in primary navigation bar.
- Click New Form button.
- Select Case Form.
- Customize rest of form as desired. See Create Dynamic Form to learn more.
- Click Save Form button.

It will display as a tab under Clients > Notes.
