- Med Rec No- Displays the Medical Record Number for the patient per service.
- Date Of Service- Displays the date of service per service.
- Duration- Displays the displays the duration of the service per service.
- Visit Type- Displays the visit type per service.
- Procedure / Units- Displays the procedure performed with the associated units per service.
- Modifiers- Displays any modifiers per service.
- Insurance- Displays the name of the insurance used by the patient per service.
- Locked Date- Displays the locked date per service.
- Provider- Displays the provider per service.
- Location- Displays the location per service.
- Transportation / Mileage- Displays the transportation method used by the patient to arrive at the location of the service and the associated mileage per service.
- Select All- Check box to select all viewable services rendered for processing.
- Unselect All- Check box to unselect all viewable services rendered.
- Test Batch- Create a test batch prior to submission.
