Request New ERAs

To receive ERAs, we request a connection from the system and our clearinghouses to the payer.

Submit the EDI-ERA Request Form

1. Click the following link to open the EDI-ERA Enrollment Request Form:  https://ensorahealth.tfaforms.net/50
2. Complete the required fields of the form, ensuring that "Procentive" is selected within the Product field. 
3. Attach any files applicable to the request.
   • If any errors are present in the form fields, an error message will appear at the top of the form window. Click the white See Errors button to navigate to the errors in need of correction.
4. Click the green Submit button within the form to submit the ERA request.

After submitting the form, allow a few business days for the dedicated Enrollments team to reach out.
