In Catalyst, the header for SOAP Notes is automatically pulled from the system for you. Please see below to see where each piece pulls from in Catalyst.

SOAP Note Header & Where Information Pulls From

1. All information from the first line is pulled from the Student | Setup | Demographics | Basic Information section.
   
    
2. All information from the second line is pulled from the details entered on the SOAP Note from the rendering provider in the App or Portal. ​
   
    
3. The Authorized ABA Supervisor pulls from the Student | Setup | Demographics | Organization/Family Information section.
