To allow a parent/guardian to sign a SOAP Note in the Portal you must make sure you have the proper field added to your template and have the proper Security Group settings.
Note: You can create a new SOAP Note template or edit an existing SOAP Note to add this special field. This specific feature only applies to how parents/guardians can now sign SOAP notes in the Portal.

Create a SOAP Note with a Parent/Guardian Signature Field
- Go to Administration.
- Select SOAP Notes | Create Template.
Enter the name of your template and click Create.
- Select + Add Field.
- Enter the Entry Text (name of the field).
Select Signature from the Answer Types dropdown.
- Click the box Guardian Signature to designate this a guardian signature field. Click Save.
Note: It is advised to leave the checkbox for Is Required? unchecked, as that will make it impossible to Publish your SOAP Note until the Guardian Signature is entered.
You will receive a pop-up confirming your success.
Edit a SOAP Note to Add a Parent/Guardian Signature Field
Note: If you already have an existing SOAP Note template with a Signature Field, you can simply Edit that Field in the template and check the box designating it as a Guardian Signature.
- Go to Administration.
- Select SOAP Notes.
Click on the three dots [...] to Edit your desired SOAP Note Template.
- Select + Add Field.
- Enter the Entry Text (name of the field).
Select Signature from the Answer Types dropdown.
- Click the box Guardian Signature to designate this as a guardian signature field. Click Save.

You will receive a pop-up confirming your success.
Create/Edit Security Group to Grant Parent/Guardian Signature Permission
- Go to Security Groups (in the Administration menu).
- Either Create Group or Edit an existing Security Group for parents/guardians.
- Check the box for Can Sign Guardian Signature Fields under SOAP Notes.
- You will want to confirm that these other permissions are also included in this Security Group.

What the Parent/Guardian will See
The Parent/Guardian will see the Actions next to the SOAP Note. Clicking on this will show the option to Sign.
